Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Most insurancs are accepted, including but not limited to Aetna, Florida Blue, Cigna, Medicare, Tricare, and United/Optum.  Please check your coverage carefully by asking the following questions:

What is my health insurance coverage and benefits?
What is my deductible and has it been met?
Do I have a co-pay? If so, how much is it?
 

Having insurance coverage is not a requirment; please inquire about our self pay rates.

Payment
Cash, check and all major credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled appointment for evaluation and you have not notified us at least 48 hours in advance, the $50 cancellation fee will be charged.  The fee for late cancellation/no show for follow-up appointments is $35.
